Medication

Dear Parent/Guardian,

For the protection and well being of all students, all medications (Prescription and Non-prescription) must be transported to and from school by a parent or guardian. Students are not allowed to have medication in their possession with the exception of emergency medications. Students who carry emergency medication must have a physician and parent signed permit on file with the nurse to carry specific medication at school. A permit to administer medication must be signed by a parent or guardian (may be obtained in nurse’s office) upon delivery of new medication or a change in dosage, route or frequency. Prescription medications will be counted by the nurse in the presence of the parent or guardian.Prescription medication must be labeled to include student’s name, doctor’s name, dosage and frequency of administration and must be in original container. Non-prescription medication must also be in the original container and not be past the labeled expiration date.
